To begin take I-95 to the 45th St exit, head east on 45th Street.
Make an immediate left turn on to Corporate Way. Make a left into Mizner Place.
Go to the back of the complex. Make a right, look for Suite 301 on your right side.
Palms West Office
To begin take Take I-95 to Southern Blvd., Head west past State Road 441.
Approximately 5 miles beyond 441 is Palms West Hospital.
The office is located behind the Hospital in building 1, Suite 115
